Emergencies happen at any time, any place, and at any age. Are you prepared? Can you name all of the medications and dosages you take, including over-the-counter drugs? When was your last tetanus shot? Can you list all of your surgeries and the dates? These questions are frequently asked at medical appointments. Accurate answers are of utmost importance because doctors base their diagnoses on what they are told. Many patients rely on their memory, forgetting important facts, especially in emergency situations.

"My Medical Assistant® --An Organizer for Personal Medical Information" is a place to record most of the facts people are asked on a medical questionnaire. The book consists of 128 pages in a 9” x 6” loose-leaf binder. There are 35 forms, including a medication list, medication schedule, hospitalizations, doctor’s notes, family medical history, dental and optical information, and insurance specifics and many more. Three formats are available.

"My Medical Assistant®," published in 2004, is easy-to-use, flexible, and uses layman terminology. I have combined my health care expertise and keen organizational skills to create a book that is well organized and thorough. I have personally manufactured, marketed, and sold more than 7,000 copies of the book to individuals, bookstores, pharmacies, insurance companies, hospital gift shops, and through the Internet (www.Amazon.com and www.MyMedicalAssistant.com).

In April 2008 it appeared in 'Support Plus,' a health care catalog. 'Today’s Health and Wellness Magazine' conducted a market survey with their membership in 2006. "My Medical Assistant®" received the magazine’s Seal of Approval. Their testers identified a definite need for a medical organizer in book format; it does not require passwords or electricity. Some of the membership’s comments were:
